Walmart Associate Medical Plan Member ID Cards

Each Walmart carrier's associate medical plan Member ID card is displayed below. The Member ID is identified and the value needed to enroll in myAgileLife's diabetes management program with reduced medication costs is highlighted. If there is a 'W' at the end of the Member ID, please be sure to include it when you enroll. 

When enrolling, enter the information for the specific family member who is joining the program and will receive the reduced copays. Please ensure the highlighted Member ID is entered exactly as it appears on the Member ID card (including the 'W' if one is shown on the card). Enter the enrolling member's first and last name exactly as it appears on the card. If the enrollee's name is not on the Member ID card, or the card is unavailable, enter it exactly the way it was submitted on the Walmart Associate Benefit Enrollment Form.
